AMAX e-RMA Request Form

If you need to return products to Amax, you must begin here by filling out this online form to request an RMA number. Product must have been purchased from Amax and be within warranty period. If you have questions about the warranty, and/or our return policies, please see our Terms and Conditions of Sale and Return Policies first. Or you can email our Customer Service department at customer_service@amax.com. All information must be filled out completely in order to process this request.

Dear Customer:

Your Return Merchandise Authorization number ( RMA# ) for the above referenced parts will be emailed to you. This RMA# is valid for 15 days from the date issued. Returns must be received by us within that time. After 15 days, a new RMA# must be obtained.

Please see our RMA shipping page for information on how to ship RMA products to us. After RMA product is received it will be processed and your product will be handled accordingly.

PLEASE make sure that the RMA Shipping Label is being used on each box you return to us. Package without the RMA Shipping Label on the outside will be refused.

Return for Warranty Repair / Replacement: Please ship only the defective part. Do not ship accessories, manuals, cables, diskettes, etc. unless otherwise instructed. Limited Warranty terms and conditions apply (see back of invoice).

Out of Warranty Repair: Please ship only the defective part. Do not ship accessories, manuals, cables, diskettes, etc. unless otherwise instructed. You will be charged $45.00 per hour, plus parts, for this repair. Minimum charge is $45.00. You are responsible for freight charges to and from amaxres.com.
